Early 1900s: Life Was Shorter – And So Was Work

At the start of the 20th century, average life expectancy in the UK sat around 45–50 years. Retirement was rare, brief or even nonexistent. Most people worked until their bodies could no longer cope with consistent strain – strength, mobility and long-term vitality were luxuries beyond the reach of most.

The concept of ‘healthy ageing’ held no cultural weight because ageing itself was not guaranteed. Infectious disease, poor sanitation and limited medical intervention meant that reaching old age was an achievement in itself. There was no framework for thinking about the quality of those years – only their quantity, and even that was uncertain.

The mid-20th century brought breakthroughs: antibiotics, vaccines, improved sanitation and medical care. Life expectancy rose dramatically within a just few generations and by 1950, the average UK citizen could expect to live into their late 60s. By the turn of the millennium, that figure had climbed past 75.

And while advancements in science have added years to life, little attention was paid to how those extra years might feel – physically, mentally and emotionally. The medical model focused on treating disease and extending lifespan, but the emerging gap between lifespan and healthspan – the period of life spent in good health – remained largely unaddressed.

Recent research has brought this discrepancy into sharp focus. A 2023 study published in Nature Aging found that while global life expectancy has increased substantially, the proportion of life spent with age-related disease and disability has grown in parallel, particularly in high-income countries. We are living longer, but not necessarily better.
